A member must be at least sixteen (16) years or older.

Eligible for or has Mass Health, Commonwealth Care Alliance(CCA), Fallon Health, United Health, and Tufts coverage.

Have a medical or mental condition and in need of assistance with Activities of Daily Living (ADLs) for at least one of these: Walking, eating, full body dressing, using the toilet, transfers (bed to chair).

Has Been clinically approved by primary care physician (PCP)
and Mass Health.
